This form is used to give Academy Hill permission to use your child's image in school publications and press releases. It is also the form you use to inform Academy Hill whether or not your child is allowed to use the internet while at school.

Forum involves fifth through eighth grades, so different
expectations from the GMS have been developed for the Scholars and the Sojourners. Be
sure to review these forms carefully.
New Students and
those entering grades K, 4 & 7):
All incoming students and those
entering grades K, 4 & 7 are mandated by law to have a physical andhealth/immunization form completed within the past 12 months on file.
